Currently eligible to book appointments as per DPH guidelines:

Phase 1: please bring employment ID with you ,and enter your employment information when signing up.
Phase 2 GROUP 1 ONLY - individuals ages 75 and over with photo identification indicating date of birth.
If you are in Phase 2, Group 2 (ages 65-74 with 2 comorbidities, you are NOT eligible to book an appointment at this time).

These appointments are for FIRST DOSE OF MODERNA VACCINE ONLY. If you are seeking a second dose appointment, please use the DPH website Vax Finder MA. Please check your eligibility before booking (graphic below); we rely on your honesty in this process. You must be 18 years of age to receive the Moderna Vaccine.
VACCINE CONTRAINDICATIONS - DO NOT BOOK APPOINTMENT-

You are under 18 years of age.
You have a history of severe allergic reactions to any vaccine.
You have received any other vaccine in the last two weeks.
You are in the quarantine/ isolation period after COVID exposure or illness.

These appointments are for JANSSEN (Johnson & Johnson). There is only one dose of this vaccine. please use the DPH website Vax Finder MA. You must check your eligibility before booking (graphic below); we rely on your honesty in this process. You must be 18 years of age to receive the Moderna Vaccine.

At this time we are working to determine the future of the clinic.
No new appointments will be released before April 13th.

Before you leave for your appointment:

Wear a mask,bring an ID & show up at your assigned time.

At your vaccination appointment:

Keep six feet of distance between you and others while inside and in lines.
You will get a vaccination card that tells you which COVID-19 vaccine you got and the date of your vaccination.
You will be watched for 15 to 30 minutes after your vaccination.

Important documents to print & bring with you :

COVID vaccination consent form : English, español
Employee ID proof, Valid ID and
Senior people with "Medicare Advantage Plan", Please bring both the red/white/blue and your supplemental card.
